Hospice Referral Management Software

Referral and intake management

Know where every referral is, and who has it.

However a referral arrives, it then moves through people — a liaison, an intake coordinator, a nurse, a director. Hospice CoPilot holds every open referral in one queue with a stage, an owner, a clock and what it is still waiting on, so none of them goes quiet without somebody seeing it.

How referrals go quiet

Three ways a referral stops moving without anyone deciding it should.

Waiting looks exactly like working

A referral held up on a date of birth, a face-to-face or a signature sits in the same place as one that is actively progressing. From a list, the two are indistinguishable.

Each record shows what it is waiting on.

The real queue lives in somebody’s inbox

Referrals get picked up by whoever saw them first. That usually means no single list anyone trusts as complete, and a weekly meeting spent rebuilding one.

One queue holds every open referral, whoever took it.

You find out when it is already gone

Most intake problems surface as a decline or a non-admit — weeks after the point where somebody could still have done something about it.

Overdue items surface daily, not in a monthly review.

The queue

Every open referral, grouped by the stage it is actually in.

New referral, intake review, outreach, info visit scheduled, clinical evaluation. Each card carries the referring source, the owner, the referral date, the last note and what is outstanding — so a stand-up is reading a board rather than reconstructing a week from memory.

The record behind it

A referral is attached to an account, not floating on its own.

Open the source and the pattern is there: how many referrals it has sent, how many became admissions, the conversion rate, when a rep last visited and how overdue that is. The individual referral stops being an isolated task and becomes part of a relationship somebody owns.
